Incident Notification Policy

Purpose

This document defines the legislative and statutory requirements for workplace incident notifications for which P6 Projects is responsible.

Scope and Definitions

This policy applies to all P6 Projects workplaces.

Definitions: 

Plant includes any machinery, equipment, appliance, container, implement and tool and any component of any of those things and anything fitted or connected to any of those things.

Worker means a person who carries out work in any capacity for P6 Projects, including work as an employee, contractor or subcontractor, an employee of a contractor or subcontractor, a labour hire employee, an apprentice or trainee, a student gaining work experience or a volunteer.

Notifiable incident means

  1. the death of a person; or

  2. a serious injury or illness of a person; or

  3. a dangerous incident.

Serious Injury or Illness of a person means

  1. immediate treatment as an in-patient in a hospital; or

  2. immediate treatment for;

  • amputation of any part of body

  • serious head injury

  • serious eye injury

  • serious burn

  • serious lacerations

  • spinal injury

  • the separation of skin from an underlying tissue

  • the loss of a bodily function

  1. Medical treatment within 48 hours of exposure to a substance (hazardous chemical).

Dangerous Incident means

An incident in relation to a workplace that exposes a worker or any other person to a serious risk to a person's health or safety emanating from an immediate or imminent exposure to:

  1. an uncontrolled escape, spillage or leakage of a substance

  2. an uncontrolled implosion, explosion or fire

  3. an uncontrolled escape of gas or steam

  4. an uncontrolled escape of a pressurised substance

  5. electric shock 

  6. the fall or release from a height of any plant, substance or thing

  7. the collapse, overturning, failure or malfunction, or damage to, any plant authorised for use in accordance with the regulations (see – WHS Regulations / Schedules 3,4 & 5)

  8. the collapse or partial collapse of a structure

  9. the collapse or failure of an excavation or of any shoring supporting an excavation 

  10. the inrush of water, mud or gas in workings, in an underground excavation 

  11. the interruption of the main system of ventilation in an underground excavation or tunnel

  12. any other event prescribed by the regulations, but does not include an incident of a prescribed kind.

The P6 Projects is strongly committed to all aspects of Work Health & Safety in its workplaces. This policy outlines the duties of the P6 Projects in managing all aspects of incident notification under its legislated responsibilities.
